TERMS AND CONDITIONS

FOR RESIDENTIAL AGED CARE SENIORS CARD RESPITE DISCOUNT PROGRAM

 

1          PREAMBLE

1.1       Mercy Aged and Community Care Ltd ACN 088 254 460 ABN 77 191 901 062 (MACC) wishes to offer a Seniors Card Respite Discount to eligible clients (the Seniors Card Respite Discount Program).

1.2       A client is eligible for the Seniors Card Respite Discount Program if the client:

(a)        is approved to receive Aged Care or Department of Veterans Affairs funding; and

(b)        has an aged care assessment and a referral code for funded respite; and

(c)        has funded respite available for the duration of their proposed stay; and

(d)        has not previously accessed an offer relating to respite at any Mercy Health location in the past 12 months; and

(e)        holds a valid Seniors Card in the state of Victoria at the date of applying for the Seniors Card Respite Discount Program.

1.3       The Seniors Card Respite Discount Program is offered on these terms and conditions and any additional terms and conditions published by MACC from time to time.

2          DEFINITIONS

2.1       Promotion Period means the period between 9:00am on 1 July 2024 and 11:59pm on 30 June 2025.

2.2       Seniors Card means a Seniors Card or Seniors Business Discount Card.

2.3       Seniors Card Respite Discount means the application of a 10% discount on the cost of a respite stay which is made up of a Basic Daily Care Fee plus any additional service fee that is payable. To avoid doubt, the Seniors Card Respite Discount does not apply to any other charges.

2.4       Participating Residential Aged Care Home means Mercy Place Keon Park, Mercy Place Templestowe, Mercy Place Parkville and Mercy Place Wyndham.

3          REDEMPTION PROCESS

3.1       An eligible client redeems the Seniors Card Respite Discount by:

(a)        booking a respite stay at any Participating Residential Aged Care Home; and

(b)        submitting a copy of their Seniors Card, with a clearly visible identifying number and card holder name,

within the Promotion Period.

3.2       By applying for the Seniors Card Respite Discount Program the client:

(a)        acknowledges that their participation in the Seniors Card Respite Discount Program is subject to these terms and conditions; and

(b)        consents to the collection, use and disclosure of personal information for the purposes of the Seniors Card Respite Discount Program.

3.3       The Seniors Card Respite Discount commences on the date of admission to the Participating Residential Aged Care Home provided that admission takes place before 31 June 2025.

 

4          GENERAL

4.1       A client can redeem this offer on one respite stay only during the Promotion Period.

4.2       This offer is subject to availability at any Participating Residential Aged Care Home.

4.3       The Seniors Card Respite Discount ends if the client is no longer eligible to hold a Victorian Seniors Card.

4.4       It is the responsibility of the client to inform MACC if they become ineligible to hold a Seniors Card.

4.5       Benefits arising from participation in the Seniors Card Respite Discount Program are non-transferable and may not be exchanged for cash or any other alternative benefit.

4.6       Only one Seniors Card Respite Discount can apply to a client at any given time.

4.7       MACC may amend or revoke these terms and conditions at any time.  While MACC will make reasonable attempts to notify participating clients of any amendment or revocation of these terms and conditions, it is not required to do so and may alter or revoke the Seniors Card Respite Discount Program at any time.

4.8       These terms and conditions shall be governed by and construed in accordance with the laws of Victoria and any disputes arising out of or in connection with them shall be subject to the exclusive jurisdiction of the courts of Victoria.
